HLC is grounded in a Community Health model which is rooted in community participation and support. The following Portland Clinics recognize this, value health equity, and contribute to make Healthy Living Community Possible through recurring donations. We also meet bi-annually to connect and support each other in enhancing public health, building networks & referrals, and weaving a healthier community.
